Physician Referrals

Physician referrals are accepted but not required. To refer a patient, complete our referral form using the link below. Select the Specialty Services Outpatient Referral Form. In the “Services Requested” section, please check “Adolescent Medicine/Teen Health Center”, specify “Substance Use Treatment” and include the primary substance of concern.
