How to Apply

• Please contact otfellowship@cchmc.org for application materials
• Applicants will be required to submit the following: letter of intent, resume/CV, graduate school transcript, and three letters of recommendation
• Applications are accepted from January 1st until January 31st

Pre-requisites

• Graduate of an accredited occupational therapy program
• Eligible for occupational therapy licensure in the state of Ohio prior to the start date of the fellowship. The applicant must obtain Ohio licensure at least two weeks prior to the start of the Fellowship Program

Important Dates

• Application Deadline: January 31st
• Interviews: Spring
• Start Date: July
