Programs and Services
Our goal is to work with your primary care physician(s) during this time of transition, not replace them. We are also available for one-time consultations for families and physicians who need some assistance or would like further education about this process.
Our clinic sees individuals 13 to 25 years old with mild to severe intellectual disabilities, including:
- Autism spectrum disorders
- Down syndrome
- Fragile X syndrome
- Klinefelter’s syndrome
- Prader-Willi syndrome
- Spina bifida
- Turner syndrome
- Velocardiofacial syndrome (VCFS)
- Williams syndrome
- Other genetic conditions with intellectual disabilities
Our services include:
- Patient and family education on health and wellness
- Behavior management
- Development of life skills and self-care abilities
- Transfer of care into the adult health care system
- Educational counseling and coordination between schools and the medical team
- Assistance with vocational rehabilitation services
- Connections to community services for recreation and social skills
- Financial resources counseling
- Discussion of guardianship and estate planning
