Schedule your Histiocytosis Center appointment. If a family member makes the initial contact, our team will call the child’s physician to confirm the referral and obtain additional medical information.
Physician Referrals
Physician referrals are welcome but not required. Providers can make a referral through Epic, or by calling Physician Priority Link at 513-636-7997 or 1-888-987-7997, or faxing us a referral form—available at the link below.
