Please note new Ohio immunization requirements for children entering 7th grade and 12th grade for the 2016-17 school year.
Grade 7: One dose of meningococcal vaccine (MCV4) must be administered prior to entry.<
Grade 12: Two doses of meningococcal vaccine (MCV4) must be administered prior to entry.
